WHAT WE DO

Supported Living

The main feature of Supported Living is that the housing and support arrangements are separate. What this means is that service users will have a tenancy agreement with a Registered Housing Landlord and Nexus will provide the day to day support. In all of our current supported living arrangements we provide 24 hour support.

Supported living can include you being supported in leisure social and educational activities, personal care, developing skills, maintaining and developing friendships and relationships, day to day household tasks, keeping healthy, managing money and maintaining the rules of your tenancy. Basically supporting you to live life in the way that you wish including realising your dreams and aspirations. (Link to Service user/family information)

Outreach/Floating Support

Outreach/Floating support is also referred to as ‘Domiciliary Care’ which is the name given to any support that involves the personal care of a family member whilst living in their own home.

The Outreach team offer a flexible and adaptable service to young people and adults with a learning disability, the visits are usually pre-planned activities which range from personal care to shopping, from paying rent to using public transport from attending social events to support with financial planning. The support on offer is always tailored to meet your needs and wishes, never assuming that all services should be the same. (Link to Service user/family information).

Community Day Service (New Beginnings)

The New Beginnings Service provides support to adults who have learning disabilities and complex physical and/or health needs. The purpose of the service is to offer service users the opportunity to experience a range of day time activities that will develop their skills and assist their independence.

The service runs 5 days a week (Monday to Friday), between the hours of 8.30am to 5pm.

The service operates from a ‘base’ from which you/your family member can access community activities or health services and where appropriate, develop links in order to make the transition into other services. These may include adult education, further education and employment. The ‘base’ also enables you/your family member to access appropriate personal care facilities and Physiotherapy activities appropriate to their individual needs.

The service will respond to individual need and therefore people can access the service for both short term and long term periods depending on personal circumstances. (Link to Service user/family information/document)
